Instructions for Completing the ETA 750B Application for Alien Employment Certification
You may download a copy of the ETA 750B at this link. Please print out the form and print clearly in ink or use a typewriter to complete the form. If a question does not apply to you, please write "N/A" in the space provided. Please provide two ETA750Bs with your original handwritten signature and one copy.
1. Name of Alien (Family name in capital letters)
- Please enter your full name.
2. Present Address (No., Street, City and Town, State or Province and ZIP code)
- Please enter your current address, whether in the US or outside of the US.
3. Type of Visa (If in U.S.)
- If you are in the US, please enter the type of visa that you have, i.e. J-1, F-1, H-1B, etc.
4. Alien's Birth date(Month, Day, Year)
- Please enter your date of birth. Do not use a number for the month. Please write it out, e.g. October 22, 1971.
5. Birthplace (City or Town, State or Province)
- Please enter your birthplace.
6. Present Nationality or
Citizenship (Country)
- Please enter the country of your current nationality or citizenship.
7. Address in the United States Where Alien Will Reside
- Please enter the address in the US where you live or will live.
8. Name and Address of Prospective Employer if Alien has job offer in U.S.
- If you are in the US, please enter the business name and address of your current employer. If you are outside of the US, please enter the business name and address of your prospective employer, if you have a job offer in the US.

Alien is Seeking Work
- Please enter the occupation in which you are seeking work. Acceptable entries include physicist, gastroenterologist, and electrical engineering. Entries such as "professor" or "scientific research" are not acceptable.
